STEP 1
Get our Rx Order Form. A doctor’s prescription is required for the Freestyle Libre. Fill in your personal information to have us email you the order form.
STEP 2
Look for our email with the Rx order form. Then print out the order form and take it (or email it) to your doctor..
STEP 3
Ask your doctor to prescribe the Freestyle Libre and submit the order form to us on your behalf. Submission instructions are on the order form.

Step #2 – Look for our email with the Rx Form
Once you submit your personal information to us we will email you the Rx Order form. Just open the email, print out the order form and take it (or send it) to your doctor.
STEP #3 -Talk to Your Doctor
Ask for a Freestyle Libre prescription. And for them to fill out the order form for you and submit it to us.
Order Processing
A doctor’s prescription (Rx) is required for us to fulfill your order and deliver a Freestyle Libre to your home.
Once we receive the completed Rx order form from your doctor, we will:
- Contact your insurance provider to confirm coverage.
- A SunMED representative will contact you to confirm your order and process any required co-pay.
- We will review delivery details and provide a tracking number for your order.

Will my Insurance cover this device?
Based on your current diagnosis and severity of management of diabetes, Medicare, Medicare advantage and Medicare replacement plans all cover the use of a CGM. Also, many commercial insurance plans will cover the use of a CGM as well, both for the reader and the sensors.
What Is It?
The Freestyle Libre is a continuous glucose monitor system consisting of a handheld reader and a sensor worn on the back of the upper arm.
How does it work?
The sensor uses a thin, flexible filament inserted just under the skin to measure glucose every minute.
